Submit a Claim - Instructions

If you are a member of the Settlement Class and wish to receive a payment under the Settlement, you must submit a valid Claim Form.

Please read the Notice before you complete your Claim Form. To participate in this Settlement, your Claim Form must be completed to the best of your ability, signed, and then (1) mailed and postmarked by June 16, 2026, OR (2) submitted online following the instructions below by 11:59 p.m. Eastern Time, June 16, 2026.



How to File Online:

To file online, please log in with the Unique ID and PIN from the Claim Form you may have received. After you log in, you will be taken to the correct form to fill out based on your credentials. Please carefully review the form and the information presented to you. Then, complete the required information, certify the form, and submit it.

If you wish to submit documents to support your claim, for example, any correspondence or PIP payment ledgers from Defendant(s) advising you that your PIP coverage had been exhausted, you may, but are not required, to do so.

If submitting documents, they must be clear, readable copies, must be less than 20 MB per file, and must be in one of the following formats: jpg, jpeg, png, gif, tif, tiff, doc, docx, xls, xlsx, pdf, txt, rtf, or zip. Please have this documentation ready before you start to file, as your Claim Form will not be saved if you have to come back and finish at a later time. If you are unable to upload a copy of your documents as part of the online Claim Form, you will need to mail a printed Claim Form along with your documentation to the Settlement Administrator.

How to File by Mail:

If you wish to submit a Claim Form via standard mail, you will need to provide all the information requested on the Claim Form, attach any supporting documentation, and sign and date the Claim Form. (If you did not receive a Claim Form or if you have lost yours, you may download and print the Claim Form here.

Then, mail it to the following address:
